Principle Illness Navigation

Through our clinical support team and community relationships, Kindred Community Cares offers behavioral health and substance use support services. Our Behavioral Health and Peer Support staff offer mental health case management, counseling and navigation support to ensure clients can access critical behavioral health and other supportive services whenever needed.

Principle Illness Navigation (PIN): PIN provides navigation and case management services for persons with a serious, high-risk condition that is expected to last at least three (3) months and has the likelihood of experiencing worsening health outcomes that could include hospitalization, placement in a nursing facility or even death. Examples include:Dementia, HIV/AIDS, Diabetes, Congestive Heart Failure, Serious Mental Illness, Substance Use Disorders.

Services

  • Person centered, strength-based planning to promote the development of self-advocacy skills
  • Empowering the member to take a proactive role in the development and updating of their Recovery, Resiliency, and Wellness Plan
  • Assisting in the use of positive self-management techniques, problemā€solving skills, coping mechanisms, symptom management, and communication strategies identified in the Recovery, Resiliency, and Wellness Plan
