Community Health Integration

Community Health Integration (CHI) is a Medicare Part B covered benefit that supports community health integration services to address patient needs and help providers to diagnose or treat patient’s medical conditions. KCC provides these integration services through the use of Community Health Workers (CHWs) trained to help navigate a complicated social care system. Some community health integration services include:

  • An assessment to better understand resident’s lives
  • Care coordination
  • Health education
  • Patient self-advocacy training
  • Health system navigation
  • Social and emotional support
