Kindred was created to assist communities struggling to address the impacts of social determinants of health (SDoH) and the resulting health related social needs (HRSNs) of their residents in today’s resource constrained environment.

We are a Community Care Hub serving as the central point to advocate for and coordinate social services for community residents to ensure they have access to professional, quality services that positively impacts their health, and improves health equity.
