By utilizing a Community Care Hub (CCH) model of care, Kindred functions as a central point for the efficient coordination and navigation of various community resources to meet HRSNs. Our goal is to improve health equity for low income residents by ensuring an equitable distribution and easier access to limited community services.

Kindred was established to support communities struggling to address the impact of social determinants of health (SDoH) and the resulting health related social needs related to food, transportation, and housing. Our focus is on navigating and coordinating essential social care and care management services to enhance overall community health.
