Community Care Hub: Navigating the Social Care Network Better

At Kindred Community Cares, we make accessing essential community resources to address your social and economic needs easier. Also known as "Kindred", we are a community care hub (CCH) located in South Central, VA supporting Petersburg, Colonial Heights, Hopewell, Dinwiddie, Prince George, and other surrounding areas to address the negative impacts of social determinants of health (SDoH). As a Medicare service provider, we assist eligible Medicare/Medicaid recipients to navigate the social care network to address social and economic needs related to food, transportation, and housing.

About Us

Connecting You to Vital Community Resources
As a community care hub (CCH) with a new model of care, Kindred functions as a central point for efficient coordination and navigation of various community resources for HRSNs. Our goal is to enhance health equity and improve overall community well-being. We are also a Medicare Part B provider prioritizing the needs of Medicare recipients and low-income residents requiring peer support, transportation services, and beyond.
