Community Care Hub: Navigating the Social Care Network Better

At Kindred Community Cares, we make accessing essential community resources to address your social and economic needs easier. Also known as "Kindred", we are a community care hub (CCH) located in South Central, VA supporting Petersburg, Colonial Heights, Hopewell, Dinwiddie, Prince George, and other surrounding areas to address the negative impacts of social determinants of health (SDoH). As a Medicare service provider, we assist eligible Medicare/Medicaid recipients to navigate the social care network to address social and economic needs related to food, transportation, and housing.
